Certification & Misc. Fees	Each instructor must provide the bureau with the names, addresses, dates of birth and driver’s license numbers, state I.D. numbers or Social Security numbers of the students of the students who successfully complete the course along with their date of completion. The instructor must also remit $3 for each name submitted.
